In the wake of highly publicized duodenoscope-associated outbreaks caused by multidrug-resistant organisms’ herculean efforts were made in order to understand the circumstances that led to these transmission events. Although there has been accumulated research that has helped to provide a clearer picture on how these outbreaks happened there are still significant data gaps when it comes to understanding the rate of duodenoscope-associated infections (DAI) especially in non-outbreak situations. Given this data gap it is important to identify those patients that are at risk of post-procedure infection so that appropriate infection control measures may be implemented. Patient segmentation can be approached using patient, procedural and duodenoscope-associated factors that may increase the risk of infection so that endoscopists can make informed decisions about the risk benefits of using enhanced duodenoscope technologies in the care of their patients.
